On this page, you will find the link to our online new patient paperwork complete with instructions for completion. We ask that you dedicate time to fill out your health history in its entirety and return it to the office via email within the next 24 hours. This allows our team of doctors to begin reviewing your case. In addition, if we do not have the new patient paperwork 48 hours before your appointment time, we will unfortunately not be able to hold the appointment and you will be charged a $75 non-refundable fee.
To optimize your time with the doctor, we ask that you arrive 30 minutes before your scheduled time with the doctor so that we can perform preliminary testing. We pride ourselves in helping patients achieve incredible results as well as being as timely as possible with our appointments.
